Cancellation Request Form

Did you know that New York Yoga can freeze your membership? If you would like to put your membership on hold for one-three months instead of cancelling, please fill out the Freeze Request form instead.

Cancellation requests must be submitted 30 days prior to monthly billing date.

By submitting this request, I understand that this request must be submitted 30 days prior to my monthly billing date. If I am terminating my annual membership prior to my 12 month commitment being met, I understand that I am subject to a $250 Early Cancellation Fee unless proper documents (e.g. doctor's note, etc) have been submitted. Additional materials to support my request (e.g. doctor's note, etc) must be faxed to 212-717-0706.
